This document explains when and why we collect personal information about people, how we use it, the conditions under which we may disclose it to others and how we keep it secure. 

Revival North Yorkshire CIC (Revival) will: 

  • only process information necessary to establish or maintain membership or support only process information necessary to provide or administer activities for people who are members of the organisation or have regular contact with it 
  • only hold information about individuals whose data is needed 
  • only hold and process the personal data information that is necessary. 

Revival is committed to ensuring your privacy is protected. Any information by which you can be identified, will only be used in accordance with this privacy statement. 

The processing of personal data is governed by the UK General Data Protection Regulation (UK GDPR). 

Using the definitions stated in the GDPR, Revival North Yorkshire CIC is the Data Controller. This means it decides how your personal data is processed and for what purpose. Revival’s ICO (Information Commissioner’s Office) registration number is: C1517572. 

Revival complies with its obligations under the GDPR by keeping personal data up to date; by storing and destroying it securely; by not collecting or retaining excessive amounts of data; by protecting personal data from loss, misuse, unauthorised access and disclosure and by ensuring appropriate technical measures are in place to protect personal data. 

How do we collect information from you? 

We collect information from you when you contact us about our services, to apply for employment or to become a volunteer, to make a donation or if you register to receive our newsletter. 

What type of information is collected from you? 

We currently collect and process the following information: 

  • Personal identifiers such as first name, last name, title, date of birth and other identifiers that you may have provided for the purpose of employment, volunteering or joining as a Revival participant. 
  • Your contact information such as address, email address, telephone numbers and any other information you have given to us for the purpose of employment, volunteering or joining as a Revival participant. 
  • Information that is captured on our application forms to assess suitability of our services, employment or volunteering, to enable us to contact individuals appropriately or keep people informed about our news and events. 
  • Special personal information is data about your race or ethnicity, religious or philosophical beliefs, sex life, sexual orientation, political opinions, trade union membership, information about your health and genetic and biometric data – we may collect certain special personal information about you if there is a lawful basis on which to do so. 
  • We may also aggregate anonymous data such as statistical or demographic data for any purpose. Anonymous data is data that does not identify you as an individual. Aggregate data may be derived from your personal data but is not considered personal in law because it does not reveal your identity. 

Your personal data – what is it? 

Personal data relates to a living individual who can be identified from that data. Identification can be by the information alone or in conjunction with other information in the Data Controller’s possession or likely to come into such possession. 

How do we get the personal information and why do we have it? 

The information you provide will be processes in accordance with the UK General Data Protection Regulations (UK GDPR) and the Data Protection Act 2018 and will be used for the following purposes: 

  • To enable Revival to create electronic and paper records of your needs. 
  • To enable Revival to support you and your needs to be arranged. 
  • For employee and volunteering recruitment and human resource purposes. To enable Revival to compile for statistical and reporting purposes. 

The information you provide to us will be stored and processed securely on our internet cloud record system and when appropriate in a locked and secure filing cabinet. 

Most of the personal information we process is provided to us directly by you for one of the following reasons: 

  • Processing a job or volunteering application 
  • Processing a Revival participant application 
  • Seeking your views or comments 
  • Sending you communications you have requested and that may be of interest to you. These may include information about our services, events and activities 
  • Notifying you to changes to our services 
  • Carrying out our obligations arising from any contracts entered into by you and us. 

We may also receive personal information indirectly from Health and Social Care Services, Community Support Organisations, Housing Agencies, and other support agencies for the purpose of a request for support or services. 

We review our retention periods for information on a regular basis. We will hold your personal information on our systems for as long as is necessary for the relevant activity, or as long as it is set out in any relevant contract or agreement with us. 

Who has access to your information? 

We will not share or sell your information with third parties for marketing purposes. 

Data will be shared with organisations that we have a formal relationship with through a contract or data sharing agreement or to comply with a statutory requirement. 

We may also share your personal information with Health and Social Care Services, Community Support Organisations, Housing Agencies, and other support agencies for the purpose of signposting or a request for other support or services. 

We will always ask for your specific consent to sharing before it happens. (Except in such cases of for example serious risk to life or potential illegal activity where we may need to share your information without your consent). If you do not consent to the sharing of data we may not be able to support you. 

Your choices 

We will not contact you for marketing purposes by email, phone or text message unless you have given us your prior consent. We will not contact you for marketing purposes by post if you have indicated you do not wish to be contacted. You can change your marketing preferences at any time by contacting us by email: contact@revivalnorthyorkshire.com. 

Your data protection rights 

Under data protection law, you have rights including: 

Your right of access – You have the right to ask us for copies of your personal information. 

Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing – You have the right to object to the processing of your personal information in certain circumstances. 

Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances. 

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
